Job summary

Good Shepherd Health Care is seeking a Care Coordinator to support outpatient care across the continuum. The role collaborates with providers, patients, families, and community resources to assess needs, develop care plans, and facilitate follow-up, ensuring timely and appropriate care.

The position may involve direct patient care tasks during staffing shortages and requires strong clinical knowledge, communication, and the ability to work in a fast-paced environment.

Qualifications

• High school diploma or equivalent required.
• MA or LPN preferred but not required.
• BLS certification required.
• Clinical experience in outpatient care setting preferred.
• Experience with patient care plans, authorizations, or community health resources.

Responsibilities

• Care coordination & planning with providers, patients, families, and care teams.
• Coordinate referrals, tests, and follow-up appointments for timely care.
• Track care plan goals and document progress in the EHR.
• Serve as a point of contact for patients regarding treatment plans and services.
• Assist with health education and resource navigation for patient engagement.
• Obtain prior authorizations for medications and procedures in collaboration with payors.
• Document interactions and care coordination steps in patient charts.
