Job summary

Life Care Centers of America is seeking an Occupational Therapist (OT) at a Skilled Nursing Facility to provide direct rehab services upon physician referral. You will collaborate with a multi-disciplinary team to plan and implement OT treatment plans, focusing on daily living activities and functional progress for residents in a supportive environment.

The role requires NBCOT certification, state licensure, and ongoing CPR.

Qualifications

NBCOT certified upon hire; renewal optional
Graduation from an accredited OT program (BSOT or MSOT)
Currently registered/licensed in applicable State and maintain active license
One year experience preferred; new grads welcome to apply
CPR certification upon hire or during orientation and must remain current

Responsibilities

Follow occupational treatment plans for patients (activities of daily living)
Establish, assess, and modify goals that are realistic and measurable
Oversee and evaluate care given by OTAs, Rehab Aides, and students
Chart appropriately and timely
Utilize therapy software accurately
Provide excellent customer service and maintain a positive attitude toward patients
Demonstrate proper body mechanics and safety
Communicate effectively on an interdisciplinary team
