South Pacific Rehab Services is seeking an Occupational Therapist - PT in Santa Rosa, CA. The role offers hourly compensation of $60.00–$62.00 and requires active licensure/registration as an OT.

Qualifications

• Graduate of an accredited university with a B.S., M.A., M.S., or Doctorate in Occupational Therapy recognized by AOTA.
• Nationally certified by NBCOT as an OTR (if state mandated).
• Current license/registration as an Occupational Therapist in-state where applicable.

Responsibilities

• Provide a comprehensive occupational therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form timely and accurately.
• Develop a comprehensive treatment plan with long/short-term goals, frequency, and modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications, and discharges.
• Deliver treatments using appropriate modalities and adjust as needed with safety precautions.
