Job summary

South Pacific Rehab Services is seeking an Occupational Therapist - PT to join our team in Santa Rosa, CA. The role requires a licensed OT and involves performing comprehensive evaluations and delivering therapy per physician orders.

You will create detailed treatment plans, document progress, and coordinate with MDs for orders and discharges. The position offers competitive hourly pay and opportunities for professional growth within Enhance Therapies family.

Qualifications

• Graduate of an accredited university with a B.S., MA, MS or Doctorate in Occupational Therapy recognized by AOTA.
• OT must be NBCOT-certified as an OTR when required by state law.
• OT holds a current license/registration in-state.

Responsibilities

• Perform comprehensive occupational therapy evaluations based on physician orders.
• Document findings on standardized forms accurately and timely.
• Develop treatment plans with long- and short-term goals, frequency, and modalities.
• Obtain MD orders for evaluations, treatments, recertifications, and discharges.
• Provide treatments using modalities and adjust as indicated while following precautions.
