Qualifications

• Must be licensed and/or registered to practice occupational therapy.
• NBCOT designation as an OTR (state dependent).
• Current state license/registration to practice OT.

Responsibilities

• Provide comprehensive OT evaluation based on MD orders.
• Document findings on standardized evaluation forms timely and accurately.
• Develop and implement a treatment plan with goals, frequency and modalities.
• Obtain MD orders for evaluations, treatments, recertifications and discharges.
• Deliver treatment using modalities and adjust as clinically indicated.
