Job summary

South Pacific Rehab Services, part of Enhance Therapies, is seeking an Occupational Therapist (OT) - PT to join our growing team in Santa Rosa, CA. The role requires a licensed OT with NBCOT designation where required by state.

You will perform comprehensive OT evaluations, create treatment plans, and document findings while coordinating with MD orders and ensuring proper approvals. We offer flexible schedules, 401K with company match, and online CEU credits.

Qualifications

• Licensed or eligible for licensure as an Occupational Therapist in-state
• NBCOT designation as OTR if required by state
• Current license/registration as Occupational Therapist in California as applicable

Responsibilities

• Provide a comprehensive occupational therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form timely and accurately.
• Develop a comprehensive treatment plan with long- and short-term goals, frequency, duration, and modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and adjust as needed while adhering to precautions.
