South Pacific Rehab Services is seeking an Occupational Therapist - PT in Santa Rosa, CA to join a team with 30+ years of care excellence. Applicants must be licensed or registered as an OT, and NBCOT designation may be required by state.

Qualifications

• Must be a licensed and/or registered Therapist to apply.
• OT degree recognized by AOTA.
• NBCOT designation as OTR (if required by state).

Responsibilities

• Provide comprehensive occupational therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form timely and accurately.
• Develop a comprehensive treatment plan with goals, frequency and duration.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Deliver treatments utilizing modalities and adjust as indicated with safety precautions.
