Qualifications

• Licensed OT in California (CA) to practice.
• NBCOT designation if required by state law.
• Documentation and evaluation form proficiency.

Responsibilities

• Provide comprehensive OT evaluation based on MD orders.
• Document findings on standardized evaluation forms timely and accurately.
• Develop comprehensive treatment plan with goals, frequency and duration.
• Ensure MD orders obtained for evaluations, treatments, and discharges.
• Deliver treatment using appropriate modalities and adjust as needed.
