Job summary

South Pacific Rehab Services, part of Enhance Therapies, is hiring an Occupational Therapist - PT to join our dedicated team in Santa Rosa, CA. The role offers $60.00–$62.00 per hour and requires a licensed/registered OT to apply.

You will perform comprehensive evaluations, develop treatment plans, ensure MD orders, and document findings with a focus on quality care across skilled nursing, home health, outpatient, and hospital settings.

Qualifications

• Licensed OT required to apply.
• NBCOT certification recognized by state (if mandated).
• Current license/registration as Occupational Therapist in-state.

Responsibilities

• Provide comprehensive occupational therapy evaluations based on MD orders.
• Document findings on standardized forms timely and accurately.
• Develop comprehensive treatment plans with long/short-term goals, frequency, duration, and modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Provide treatment using modalities and adapt as needed while adhering to precautions.
