Continuum Therapy Partners is seeking an Occupational Therapist for Avalon Health and Rehab in Mount Vernon, OH. This is a part-time daytime role requiring a state license in Occupational Therapy and NBCOT certification. Prior OT experience is preferred, with opportunities to impact patient-centered care.

• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
