Job summary

Continuum Therapy Partners, LLC is seeking an Occupational Therapist for a full-time daytime role at Hillsdale Health and Rehab in Charlottesville, VA. The OT will evaluate and develop individualized treatment plans to improve functional independence and educate patients and families on adaptive equipment and home safety recommendations.

The ideal candidate holds a Master’s or Doctorate in OT, an active state license, NBCOT certification, and ideally has 1 year of OT experience.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state OT License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients’ functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
