Job summary

Continuum Therapy Partners in Berlin, PA is seeking a skilled Occupational Therapist to join our daytime, full-time team at Meadow View Nursing Center. You will contribute to delivering high-quality patient care across our network.

You will evaluate and develop treatment plans, document patient progress, and educate patients and families on adaptive equipment and home safety, supporting functional independence and safety in daily activities.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy is required
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
