Continuum Therapy Partners in Elroy, Wisconsin, is seeking an Occupational Therapist to join our team at Elroy Health Services. The role involves evaluating patients, developing individualized treatment plans to improve functional independence, and collaborating with families.

This part-time daytime position requires a master's or doctoral degree in OT, a current state license, NBCOT certification, and preferred 1 year of experience.

Qualifications

Responsibilities

Occupational Therapist Requirements & Responsibilities:

• Master's or Doctorate Degree in Occupational Therapy required
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred
• Evaluates and develops individualized treatment plans to improve patients' functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
