Job summary

Continuum Therapy Partners, LLC is actively seeking an Occupational Therapist to join our team. The role focuses on evaluating patients, developing individualized treatment plans, and improving functional independence in a supportive clinical environment.

You will document progress, adjust treatments based on outcomes, and educate patients and families on adaptive equipment and home safety. VA locations are included in the assignments, with PRN daytime scheduling.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
