Job summary

Continuum Therapy Partners, LLC is seeking an Occupational Therapist at Heritage Hall Dillwyn, Dillwyn, VA. The role focuses on evaluating patients, developing individualized treatment plans, and coordinating care with families.

Responsibilities include documenting patient progress, adjusting plans based on outcomes, and educating families on home safety and adaptive equipment. PRN daytime schedule with on-site work in Virginia will apply.

Qualifications

• Master's or Doctorate in Occupational Therapy is required.
• Active state OT license and NBCOT certification are required.
• At least 1 year of previous experience as an Occupational Therapist is preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.

Occupational Therapist Requirements & Responsibilities

• Master's or Doctorate Degree in Occupational Therapy required
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred
