Job summary

Continuum Therapy Partners is seeking an Occupational Therapist to join as a PRN clinician in Hayward, WI. You will evaluate patients, develop individualized treatment plans to enhance independence, and document progress while adjusting care based on outcomes.

Required are a Master's or Doctorate in Occupational Therapy, a current state license, and NBCOT certification. Prior OT experience is preferred. Schedule is PRN during daytime hours at Hayward Health Services.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
