PRN Daytime Schedule

Job summary

Continuum Therapy Partners is seeking a licensed Occupational Therapist for PRN daytime coverage at our Lake Country facilities in Oconomowoc, WI. You will assess patients, develop individualized plans, and document outcomes to support functional independence.

You will educate families on adaptive equipment and home safety, participate in care planning with a supportive team, and maintain licensure while delivering high-quality patient care in a flexible schedule.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state OT license; NBCOT certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
