Job summary

Continuum Therapy Partners is seeking Occupational Therapists to join a clinician-led team dedicated to a patient-centered therapy environment. The role focuses on evaluating patients, creating individualized plans, and supporting families with equipment and home safety guidance.

This position is PRN with daytime shifts, located at Heritagespring in West Chester, OH. Candidates must hold a Master’s or Doctorate in Occupational Therapy, current OT licensure, and NBCOT certification; a minimum of

Qualifications

• Master’s or Doctorate in Occupational Therapy required.
• Current, unencumbered state OT license; NBCOT certification required.
• 1 year of prior OT experience preferred.

Responsibilities

• Evaluate and develop individualized treatment plans to improve patients’ functional independence.
• Document patient progress and modify treatment plans based on outcomes.
• Educate patients and families on adaptive equipment and home safety recommendations.

Occupational Therapist Requirements & Responsibilities

• Master’s or Doctorate Degree in Occupational Therapy required
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred
• Evaluates and develops individualized treatment plans to improve patients’ functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
