Job summary

Continuum Therapy Partners, LLC is seeking an Occupational Therapist to join our growing team at Heritagespring - West Chester OH. The role focuses on evaluating patients, developing individualized treatment plans, and supporting functional independence in a client-centered environment.

This position requires a Master’s or Doctorate in Occupational Therapy, an active state OT license with NBCOT certification, and at least one year of OT experience; PRN daytime/weekend scheduling is available to

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
