Job summary

Continuum Therapy Partners, LLC is seeking an Occupational Therapist to join a therapist-led model focused on scalable, responsive therapy across a growing national footprint. The role emphasizes patient-centered care and collaboration with leadership to advocate for professional perspectives.

Responsibilities include developing and adjusting treatment plans, documenting progress, and educating patients and families on safety and equipment in a home setting.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy is required.
• NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.

Occupational Therapist Requirements & Responsibilities

• Master\'s or Doctorate Degree in Occupational Therapy required
