Continuum Therapy Partners is seeking an Occupational Therapist to join a therapist-led team as we expand across a national footprint. You will evaluate patients and develop individualized treatment plans to improve functional independence and guide home safety and adaptive equipment needs.

Based at Menominee Health Services in Menominee, MI, the role offers daytime schedules with either full-time or part-time options and a comprehensive benefits package including medical, dental, and vision

Qualifications

• Must have a Master's or Doctorate in Occupational Therapy.
• Current, unencumbered state OT license; NBCOT certification required.
• 1 year of OT experience preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.

Occupational Therapist Requirements & Responsibilities:

• Master's or Doctorate Degree in Occupational Therapy required
