Job summary

Continuum Therapy Partners, LLC in Wisconsin seeks an Occupational Therapist to provide patient-centered care at Sauk County Health Care Center in Reedsburg. You will evaluate and develop individualized treatment plans to improve functional independence, and collaborate with leadership that values your professional perspective.

The role requires a Master’s or Doctorate in OT, an active state license, and NBCOT certification; 1 year of experience is preferred.

Qualifications

• Master’s or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.

Occupational Therapist Requirements & Responsibilities
