Job summary

Continuum Therapy Partners is seeking an Occupational Therapist to join a therapist-led team driving a patient-centered care model. The role focuses on developing and implementing individualized treatment plans to improve functional independence, with ongoing documentation of progress and adjustment of plans as needed.

The ideal candidate holds a Master’s or Doctorate in Occupational Therapy, an active state license, and NBCOT certification.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy.
• Active state OT license is required.
• NBCOT certification is required.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.

Occupational Therapist Requirements & Responsibilities:

• Master's or Doctorate Degree in Occupational Therapy required
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred
• Evaluates and develops individualized treatment plans to improve patients' functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
