Job summary

Carle Health seeks a licensed Physical Therapist to create and modify orthotics and to evaluate patients of all ages, adjusting plans of care as needed. This role serves as a clinical resource for staff and students and emphasizes safe techniques and patient engagement.

The ideal candidate will educate patients and families, document outcomes, and collaborate with the care team to optimize rehabilitation results. A sign-on bonus and relocation assistance are available.

Qualifications

• Bachelor's Degree in Physical Therapy
• Licensed Physical Therapist (PT) in IL
• BLS within 30 days - AHA
• Complete required CE for acute stroke care

Responsibilities

• Transfers with assistance and maintain safe body mechanics.
• Treat patients with dignity, empathy, and engage with those needing services.
• Design and adjust Plan of Care based on goals, treatment, and outcomes.
• Educate patients, families, and caregivers using effective teaching methods.
• Evaluate findings to determine PT diagnosis and outcomes of care.
• Perform examinations with reliable methods based on history and screening.
• Document in EMR per CTSCP67 guidelines.
• Screen patients to determine therapy needs and effectiveness.
• Supervise support personnel and provide feedback.
• Complete annual education for DNV Hip and Knee programs if applicable.
• Evaluate impairments and establish plans of care based on functional deficits.
• Complete objective testing for ROM and strength; document daily activities.
