Qualifications

• Bachelor's Degree in Physical Therapy required.
• Licensed Physical Therapist (PT) in good standing (IDFPR).
• BLS certification within 30 days of hire.
• Attend required continuing education on acute stroke care.

Responsibilities

• Designs plan of care integrating goals, treatment and discharge plan.
• Educates patients and families using effective teaching methods.
• Evaluates and documents PT diagnoses and outcomes.
• Performs examinations with reliable methods relevant to chief complaint.
• Documents in EMR and adheres to policy CTSCP67.
• Evaluates orthopedic and neurologic impairments and plans care accordingly.
