Mercy Clinic Orthopedics Southfork in St. Louis, MO is seeking a compassionate Registered Nurse (RN) Nurse Navigator to guide patients through orthopedic care from first consultation through surgery, recovery, and follow-up.

You will coordinate care with surgeons, therapists, and teams, provide education to patients and families, and ensure timely, high-quality care while maintaining accurate documentation in EPIC.

Qualifikationen

• Registered Nurse with 2-3 years of nursing experience.
• Graduate from an accredited school of nursing; current RN license.
• BLS certification required; ACLS preferred.

Aufgaben

• Coordinate and navigate orthopedic patient care from evaluation to recovery.
• Educate patients and families on diagnoses, procedures, and post‑op care.
• Facilitate communication among patients, providers and care teams.
• Develop individualized care plans with physicians and teams.
• Support pre-operative planning, discharge coordination and follow-up.
• Monitor patient progress and address barriers to care.
• Document in EPIC and other clinical systems.
• Track outcomes and participate in quality improvement.
