Job summary

Mercy Clinic Orthopedics Southfork in St. Louis, MO, seeks a compassionate Registered Nurse (RN) Nurse Navigator to guide patients through the orthopedic care journey from initial consult to recovery.

You will coordinate care with surgeons, APPs, therapists, and care teams, provide education, discharge planning, and follow-up, and monitor outcomes to ensure timely, high-quality care. This full-time role includes a supportive team, strong benefits, and opportunities to impact patient experience.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in Missouri.
• 2-3 years of nursing experience.
• BLS certification.
• Preferred BSN and ACLS certification.

Responsibilities

• Coordinate care for orthopedic patients through evaluation, treatment, surgery, and recovery.
• Educate patients and families about diagnoses, procedures, and post-operative care.
• Facilitate communication among patients, providers, and care teams for seamless care coordination.
• Collaborate with physicians and interdisciplinary teams to develop individualized care plans.
• Support pre-operative planning, discharge coordination, and post-operative follow-up.
• Monitor patient progress and address barriers to care.
• Maintain accurate documentation in EPIC and other clinical systems.
• Track outcomes and quality metrics to support evidence-based practice.
