Qualifications

• RN license in the practicing state is required.
• BLS certification is required.
• 2–3 years of nursing experience preferred.

Responsibilities

• Coordinate and navigate orthopedic patient care from initial evaluation through discharge.
• Educate patients and families about diagnoses, procedures, and post-operative care.
• Facilitate communication among patients, providers, and care teams for seamless care.
• Collaborate with physicians and interdisciplinary teams to develop individualized care plans.
• Support pre-operative planning, discharge coordination, and post-operative follow-up.
• Monitor patient progress and address barriers to care.
• Maintain accurate documentation in EPIC and other clinical systems.
• Track outcomes and quality metrics to support evidence-based practice.
• Participate in quality improvement initiatives and program development.
• Serve as a clinical resource for patients, families, providers, and staff.
