Job summary

Mercy Clinic Orthopedics Southfork is seeking a compassionate RN Nurse Navigator to guide patients through the orthopedic care journey, coordinating evaluation, treatment, surgery, and recovery. You will educate patients and families, coordinate with clinicians, and support discharge planning from initial consult to follow-up care.

You will collaborate with surgeons, APPs, therapists, and teams to ensure timely, high-quality care while maintaining strong patient experiences.

Qualifications

• Graduate of an accredited school of nursing.
• Current Registered Nurse (RN) license in the state of practice.
• 2-3 years of nursing experience.
• Basic Life Support (BLS) certification.

Responsibilities

• Coordinate and navigate care for orthopedic patients throughout evaluation, treatment, surgery, and recovery.
• Educate patients and families regarding diagnoses, procedures, treatment plans, and post-operative care.
• Facilitate communication between patients, providers, and care teams to ensure seamless care coordination.
• Collaborate with physicians and interdisciplinary teams to develop individualized care plans.
• Support pre-operative planning, discharge coordination, and post-operative follow-up.
• Monitor patient progress and address barriers to care.
• Maintain accurate documentation in EPIC and other clinical systems.
• Track outcomes and quality metrics to support evidence-based practice.
