Job summary

Qualifications

• Bachelor's Degree in Physical Therapy required.
• Licensed Physical Therapist (PT) in Illinois per IDFPR.
• Basic Life Support (BLS) within 30 days - AHA.
• Attend and satisfactorily complete required continuing education for acute stroke care.

Responsibilities

• Demonstrates ability for patient transfers with assistance and correct body mechanics.
• Treats patients with empathy and dignity, engaging with those in need of services.
• Designs a plan of care integrating goals, treatment, outcomes and discharge plan.
• Educates patients, families, and staff using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes of care.
• Per forms examinations and selects reliable exam methods based on history.
• Produces documentation per CTSCP67 policy.
• Screens patients to determine therapy needs and effectiveness.
• Oversees support personnel and provides feedback.
• Completes annual training for HIP/DNV Knee Replacement program if applicable.
• Evaluates impairments related to orthopedic and neurologic conditions.
• Establishes plans of care based on functional deficits and patient goals.
• Completes ROM/strength tests and functional tasks assessments.
• Maintains daily documentation and therapy EMR entries.
• Applies tape for stabilization or facilitation of movement.
• Administers modalities like ultrasound and interferential therapy.
