Job summary

Mercy Clinic Orthopedics Southfork is seeking a Registered Nurse (RN) Nurse Navigator to guide patients through evaluation, surgery, and recovery. You will coordinate care with surgeons, therapists, and teams, educating patients and families on plans and post-operative care.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in the state of practice.
• 2-3 years of nursing experience.
• BLS certification.

Responsibilities

• Coordinate care for orthopedic patients through evaluation, treatment, surgery, and recovery.
• Educate patients and families on diagnoses, procedures, plans, and post-op care.
• Facilitate communication between patients, providers, and care teams.
• Develop individualized care plans with physicians and interdisciplinary teams.
• Support pre-op planning, discharge coordination, and post-op follow-up.
• Maintain documentation in EPIC and other systems.
• Track outcomes and quality metrics to improve patient care.

Education

Registered Nurse (RN) license

BLS certification

BSN preferred

ACLS certification
