Job summary

Mercy Clinic Orthopedics Southfork is seeking a compassionate Registered Nurse (RN) Nurse Navigator to guide patients through their orthopedic care journey—from initial consultation to surgery, recovery, and follow-up. You will educate patients, coordinate care with surgeons and interdisciplinary teams, and support discharge planning to optimize outcomes.

You’ll work closely with physicians and partners to ensure timely, high-quality care and an outstanding patient experience in a full-time

Qualifications

• RN license in the state of practice.
• 2-3 years of nursing experience.
• BLS certification required.

Responsibilities

• Coordinate and navigate care for orthopedic patients throughout evaluation, treatment, surgery, and recovery.
• Educate patients and families regarding diagnoses, procedures, treatment plans, and post-operative care.
• Facilitate communication between patients, providers, and care teams to ensure seamless care coordination.
• Collaborate with physicians and interdisciplinary teams to develop individualized care plans.
• Support pre-operative planning, discharge coordination, and post-operative follow-up.
