Job summary

Mercy Clinic Orthopedics Southfork is seeking a compassionate Registered Nurse (RN) Nurse Navigator to guide patients through their orthopedic care journey. You will coordinate care, provide education, and support patients from initial consultation through surgery, recovery, and follow-up.

Working with orthopedic surgeons and interdisciplinary teams, you’ll ensure timely, high-quality care while delivering an exceptional patient experience.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in the state of practice.
• 2-3 years of nursing experience.
• BLS certification required.

Responsibilities

• Coordinate and navigate care for orthopedic patients through evaluation, treatment, surgery, and recovery.
• Educate patients and families on diagnoses, procedures, and post-operative care.
• Facilitate communication between patients, providers, and care teams for seamless coordination.
• Collaborate with physicians and interdisciplinary teams to develop care plans.
• Support pre-operative planning, discharge coordination, and post-operative follow-up.
• Monitor patient progress and address barriers to care.
• Maintain accurate documentation in EPIC and clinical systems.
• Track outcomes and quality metrics to support evidence-based practice.
• Participate in quality improvement initiatives and program development.
• Serve as a clinical resource for patients, families, providers, and staff.
