Job summary

Dartmouth-Hitchcock is seeking a referral coordinator to review, schedule, and track patient referrals and communicate with providers and staff to ensure timely care.

You’ll respond to patient inquiries, greet arrivals, collect forms, and coordinate with clinicians to route information and authorize external services as needed. Strong organization and attention to detail are essential.

Qualifications

• Knowledge of insurance benefit programs and medical terminology.
• Ability to effectively interact with providers, staff, patients and insurance plan representatives a must.
• Able to prioritize multiple tasks.
• Must have prior computer experience, excellent communication skills, and attention to detail.

Responsibilities

• Responds to patient calls providing general information and education.
• Greets patients arriving for appointments, assists with forms and use of technology.
• Assesses needs of patients without appointments and processes requests for prescriptions, forms, appointments or need to speak directly to clinical support.
• Reviews schedules daily for accuracy in scheduling and manages wait lists and reschedules as needed.
• Completes follow up tasks for patients as directed by the After Visit Summary.
• Receives calls, takes messages, schedules appointments or transfers calls.
• Monitors system messages and incoming faxes to determine proper action.
• Completes Authorizations and Pre Certifications for external and stat procedures.
• Reviews provider schedules to identify follow up and handles no-shows per policy.
• Supports providers and staff in addressing questions/concerns.
• Monitors waiting room for distressed patients and uses service recovery tactics.
• Manages in-baskets by routing messages and completing tasks.
• Acts as liaison between primary care physicians & specialists outside DH.
• Monitors referral work queues and processes new referrals.
