Neighborhood Healthcare PACE seeks a Social Work Manager to provide leadership and strategic direction for social work services within the PACE program. The role ensures high-quality, person-centered psychosocial care while complying with state and federal regulations, collaborating with leadership, clinical teams, and community partners.

This onsite position runs Monday–Friday, 8am–5pm. Responsibilities include overseeing staff performance, ensuring workflow efficiency, advancing IDT processes,

Qualificações

• Three years of social work experience with elderly and frail adults and families required.
• Two years of supervisory experience required; experience with elderly population preferred.
• California Associate Clinical Social Worker (ACSW) – Preferred.
• Strong knowledge of CMS and DHCS regulatory requirements preferred.

Responsabilidades

• Provides leadership, supervision, and support to Social Work staff, fostering accountability and collaboration.
• Monitors staffing patterns, caseload distribution, and service coverage for efficiency.
• Oversee documentation quality, audit readiness, and adherence to internal standards.
• Promotes trauma-informed care, workforce development, and cross-disciplinary collaboration.
• Ensures compliance with state and federal regulations including CMS and DHCS.
