Job summary

myPlace Health in Los Angeles, CA is seeking a licensed MSW for a full-time onsite role within the PACE program. You will plan, organize, and deliver social work services tailored to each participant, coordinating care with the IDT and supporting families to help seniors age in place.

Responsibilities include psychosocial assessments, care plan conferences, transitions between settings, Medi-Cal support, EMR documentation, and discharge planning. Schedule is Monday–Friday, 8am–5pm onsite.

Qualifications

• MSW from an accredited university and licensure or eligibility.
• 2 years of psychosocial assessments, care planning, and case management.
• 2 years of experience with frail or elderly populations.
• California Driver's License with clean driving record.
• CPR and First Aid certification (or able to obtain within 30 days).

Responsibilities

• Engage in daily IDT meetings and conduct psychosocial assessments.
• Present assessments at care plan conferences for collaborative plans.
• Guide participants through care transitions between settings.
• Connect participants and families with community resources.
• Document care plans and interactions in the EMR promptly.
• Provide discharge planning support and ongoing support.
