Neighborhood Healthcare is seeking a Social Worker for the PACE program in Riverside to manage a caseload, perform psychosocial assessments, and collaborate with an interdisciplinary team to improve health and quality of life for participants over age 55.

The onsite, full-time role requires strong communication, coordination with families and providers, and adherence to documentation standards to support ongoing care and eligibility. Previous elder-care experience is preferred.

Competenze

• One year of experience working with the elderly population.
• Experience in a community-based or geriatric program preferred.
• BLS certification required upon hire; CPR and First Aid certifications renewed annually.

Mansioni

• Manage a caseload of PACE participants and provide psychosocial assessments.
• Collaborate with the IDT to optimize health status and quality of life.
• Coordinate family conferences and discharge planning.
• Assist with advance care planning and educational resources for participants and families.
• Maintain timely, federally compliant documentation in medical records.
• Attend required trainings and staff meetings.
