Job summary

On Lok PACE seeks a Social Work Lead to provide direct case management and serve as SME for the social work department. The role includes oversight, mentorship, and coordination within the Interdisciplinary Team to support participants and families.

Responsibilities include assessments, treatment planning, referrals, and community resource coordination, under supervision from managers. Strong leadership and multilingual skills are highly valued in a fast-paced, mission-driven nonprofit setting.

Qualifications

• MSW degree required or equivalent.
• Three years' experience in social work (minimum).
• Leadership experience in a team setting preferred.
• Excellent communication and interpersonal skills required.
• Experience with diverse populations and interdisciplinary teams is valued.

Responsibilities

• Provide direct case management to participants and families.
• Oversee assessments, care planning and referrals.
• Coordinate with IDT and community resources.
• Lead in onboarding and coaching Social Workers and Aides.
• Maintain documentation and ensure compliance with policies.
