Job summary

myPlace Health, a PACE organization backed by SCAN Group, seeks a Behavioral Health Manager onsite in downtown Los Angeles. Lead a multidisciplinary Social Work team delivering biopsychosocial care, support high-risk seniors, and ensure regulatory compliance within the PACE model.

Qualifications

• MSW from CSWE-accredited program and active California LCSW license.
• Minimum 5 years progressive social work experience, with 3+ years in leadership/management.
• Experience serving older adults and high-risk seniors in healthcare or senior-care settings.
• Ability to balance strategic planning with day-to-day care delivery.
• Excellent communication and collaboration across multidisciplinary teams.

Responsibilities

• Oversee Social Work department with clear goals, priorities, and performance standards aligned to mission and PACE requirements.
• Supervise and mentor Social Workers and Mental Health Counselors, fostering accountability and growth.
• Manage schedules, onboarding, coverage, and performance processes.
• Use data and KPIs to identify trends and drive service improvements.
• Ensure timely biopsychosocial assessments and individualized care plans.
• Maintain compliant, participant-centered workflows across multiple sites.
• Support high-acuity cases, crisis intervention, and transitions of care.
• Provide guidance on trauma-informed care and advance care planning.
• Ensure CMS/DHCS/PACE standards are upheld and audits are addressed.
• Lead EMR documentation quality and corrective action planning.
• Collaborate with Nursing, Therapy, Enrollment, and Center Operations.
