Job summary

myPlace Health is a PACE organization focused on compassionate, holistic care for older adults. As Behavioral Health Manager, you will lead a multidisciplinary Social Work team across the PACE model, guiding biopsychosocial assessments, care plans, and interventions while ensuring regulatory compliance and high-quality patient outcomes.

You will oversee data-driven improvements, mentor staff, and maintain strong partnerships with Nursing, Therapy, Enrollment, and Center Operations.

Qualifications

• MSW degree from CSWE-accredited program and active California LCSW license.
• At least 5 years of progressive social work experience, with 3+ years in leadership.
• Experience with older adults, high-risk seniors, and complex medical/psychosocial needs.
• Strategic thinker who balances long-term vision with daily operations.
• Excellent verbal and written communication and collaboration across teams.
• Commitment to health equity and inclusive care.
• Willingness to travel between sites and spend time onsite.
• Onsite schedule: Monday–Friday.

Responsibilities

• Oversee Social Work department by setting goals and performance standards.
• Supervise and mentor Social Workers and Mental Health Counselors.
• Manage team schedules, onboarding, and performance management.
• Use data and KPIs to identify trends and drive service improvements.
• Ensure timely biopsychosocial assessments and care plans.
• Refine workflows and policies for consistent, participant-centered care.
• Support high-acuity cases, crisis intervention, and transitions of care.
• Provide guidance on trauma-informed care and social determinants of health.
• Maintain a small participant caseload to stay connected with practice.
• Ensure CMS, DHCS, and PACE compliance.
• Oversee EMR documentation and lead audits and corrective actions.
• Collaborate with Nursing, Therapy, Enrollment and Center Operations.
• Represent Social Work in planning and growth initiatives.
• Manage resources and vendor partnerships for social work services.
• Perform other duties as needed.
