Job summary

myPlace Health is seeking a Behavioral Health Manager onsite at our downtown Los Angeles PACE clinic. Lead a multidisciplinary Social Work team delivering holistic, biopsychosocial care to high-risk seniors, aligned with CMS/DHCS/PACE standards.

You will supervise social workers and counselors, leverage data to improve outcomes, and coordinate across Nursing, Therapy, Enrollment, and Center Operations to ensure seamless care. A California LCSW is required.

Qualifications

• Master's degree in Social Work from a CSWE-accredited program and an active California LCSW license.
• Minimum 5 years of progressive social work experience, with 3+ years in leadership or management.
• Experience in healthcare, PACE, or senior-care settings; ability to support high-risk populations.
• Strong strategic thinking, clear communication, and collaborative leadership.

Responsibilities

• Oversee the Social Work department with clear goals, priorities, and performance standards.
• Supervise and mentor Social Workers and Mental Health Counselors, promoting accountability and growth.
• Manage schedules, onboarding, and operational rhythms while driving quality improvements.
• Use data and KPIs to inform decisions and improve service delivery.
• Ensure biopsychosocial assessments, care plans, and interventions meet standards and regulations.
• Ensure compliant, participant-centered care across sites and coordinate cross-functional care.
• Support high-acuity cases, crisis intervention, and transitions of care to protect participants.
• Lead through trauma-informed care, advanced care planning, and addressing social determinants of health.
• Maintain engagement with Nursing, Therapy, Enrollment, and Center Operations for integrated care.
• Represent Social Work in planning and growth initiatives and manage vendor partnerships as needed.
• Remain flexible, mission-driven, and travel between sites as required.
