Job summary

Reserve Care seeks an experienced Registered Nurse to provide prescribed medical treatment and personal care to terminally ill patients. You will work with a transdisciplinary team across settings to implement and coordinate each patient’s Plan of Care.

OCI-Ohio licensed nurses with CPR certification and strong communication skills are preferred. The role includes on-call rotation, discharge planning collaboration, and support for patients and families during end-of-life care.

Qualifications

• Licensed RN in Ohio required or eligible.
• Current CPR certification required.
• Valid driver’s license with good driving record.
• Hospice-palliative care experience preferred.

Responsibilities

• Assess physical, psychosocial, and spiritual needs of patients and families.
• Provide intermittent skilled nursing under physician direction.
• Monitor and manage pain and symptoms; educate patients and families.
• Coordinate Plan of Care with the transdisciplinary team.
• Participate in care conferences and bereavement follow-up.
• Assist with discharge planning and interdisciplinary communication.
