Job summary

Reserve Care is seeking a Clinical Admissions Nurse to assure accurate representation of hospice programs to patients and families through assessment and admissions in home, facility, or healthcare settings. You will work closely with Referral Department to promote the hospice philosophy of care.

The role requires Ohio RN licensure, a BSN and/or CHPN preferred, and at least one year of nursing experience in intake, home care, hospice or oncology nursing; valid driver's license is required.

Qualifications

• Graduate of an accredited nursing program with BSN and/or CHPN.
• Experience with volunteer management and HR practices preferred.
• Ohio RN licensure required and valid driver9 license.
• Minimum of one year nursing experience in intake, home care, hospice and/or oncology nursing.

Responsibilities

• Performs all duties and responsibilities in accordance with the Nurse Practice Act and professional nursing principles.
• Provides information assessment and admission services to patients referred to HWR: conducting assessment visits in home or facility as necessary.
• Communicates with Referral Department, referring Physician, and Hospice MDs to assure appropriateness of each referral and necessary certifications.
• Keeps the Referral Department informed about the progress of referral/admission.
• Applies nursing principles and techniques in treatment of patients; provides nursing care, emotional support and education to patients and families.
• Utilizes computer skills to access medical information and documents in EMR as required.
• Promotes teamwork by participating as a member of an interdisciplinary team and coordinates care.
• Completes and submits documentation within agency timelines and policies.
• Maintains HIPAA privacy standards and agency policies.
• Collaborates with other IDT members to coordinate care efficiently and financially responsibly.
• Makes referrals to other disciplines and equipment to meet patient needs timely.
• Works with diverse social/economic situations while maintaining professional boundaries.
• Meets standards for continuing education and licensure.
