Job summary

Ennoble Care is seeking a full-time Care Coordinator to join our Waco, Texas Home Office team. This on-site role serves as a key member of our interdisciplinary care team, supporting patients through proactive care coordination, collaboration, and a patient-centered approach.

This Care Coordinator works closely with fellow coordinators, providers, patients, families, and clinical staff to help ensure seamless continuity of care.

Qualifications

• MA certification with at least 1 year of clinical experience.
• Legally authorized to work in the United States.
• Proficient with EMR systems and MS Office; strong communication skills.

Responsibilities

• Coordinate and manage patient care activities within the EMR system to support continuity of care.
• Assist referrals, lab orders, follow-up, medication refill requests, and DME coordination.
• Maintain frequent communication with patients, providers, families, and care teams to address needs and ensure timely follow-up.
• Document all patient interactions accurately and report changes promptly to the clinical team.
• Demonstrate professionalism, compassion, and strong organization while managing multiple priorities in a fast-paced environment.
• Utilize EMR, secure email, Microsoft Office, and phone systems to support daily operations.
