Job summary

Ennoble Care seeks a full-time CMA Transitional Care Coordinator to join on-site teams in Waco, TX. You will manage transitional care within partnered facilities, coordinating with providers and facility staff to keep care plans current and patient-centered.

You’ll monitor residents, communicate changes, document thoroughly in the EMR, and ensure timely follow-ups. The role focuses on preventing hospitalizations while supporting families in a compassionate, team-based environment.

Qualifications

• Legally authorized to work in the United States.
• Active, unrestricted CMA license in Texas.
• Minimum 1 year of patient-facing experience in long-term care, skilled nursing or transitional care.
• Comfortable communicating in person, by phone, email, and via Microsoft Teams.
• Solid knowledge of healthcare terminology and care coordination.
• Strong clinical assessment skills with ability to identify and elevate changes in condition.
• Experience with EMR systems and clinical documentation; PCC experience preferred.
• Proficient with Microsoft 365 including Excel, Outlook, Word, and Teams.

Responsibilities

• Coordinate care for residents in skilled nursing and rehab facilities using Ennoble Care's EMR.
• Facilitate transitional care management with Housecalls upon discharge from hospital.
• Expedite time to first provider visit for newly transitioned patients.
• Monitor condition, communicate changes promptly, and elevate care as needed.
• Support care coordination to reduce avoidable ED visits and hospital readmissions.
• Collaborate daily with providers and facility staff to keep care plans current.
