Job summary

Ennoble Care in Cleveland, Ohio is seeking a full‑time Certified Medical Assistant to serve as a Transitional Care Coordinator in partnered long‑term care facilities. This on‑site role supports care transitions, patient monitoring, and collaboration with the clinical team to keep residents healthy at home. You will work 8:00 a.m.

Qualifications

• Active, unrestricted CMA license in the state of Texas.
• Minimum 1 year of patient-facing experience in long-term care or transitional care.
• Proficient with EMR systems and clinical documentation; CCM‑billable activities are a plus.

Responsibilities

• Coordinate care for patients in skilled nursing and rehabilitation facilities using the EMR.
• Facilitate transitional care management and initiate services upon discharge.
• Monitor patient condition and communicate changes promptly to the provider.
• Support care coordination to reduce avoidable ED visits and hospital readmissions.
• Collaborate daily with providers and facility staff to keep care plans current.
