Ennoble Care is seeking a full-time Embedded Care Coordinator to join our clinical team in the Leesburg, VA area. You will work on-site in long-term care facilities, coordinating care with our providers and facility staff to ensure continuous, patient-centered support.

You must hold an active clinical license (MA/CMA/LPN/RN) and have at least 1 year of patient-facing experience in a long-term care setting. Regular on-site presence, strong communication, and reliable transportation are essential.

Competenze

• Legally authorized to work in the United States.
• Active, unrestricted clinical certification or licensure such as MA/CMA/LPN/RN.
• Minimum 1 year of patient-facing experience in long-term care or similar setting.
• Proficient with Microsoft Teams, email, phone, and in person.
• Strong relationships with residents, families, and staff; dependable presence.
• Excellent organizational and time-management skills.
• Knowledge of healthcare terminology and care coordination principles.
• Proficiency with Microsoft 365 (Outlook, Word, Excel).
• Full-time, Mon-Fri 8:00 a.m. – 4:30 p.m. EST on-site in Reston, VA near Leesburg.
• No on-call; Daytime hours; 7 company-paid holidays.
• Valid driver’s license, reliable transportation, auto insurance; travel between facilities may be required.
• Ability to pass a background check.

Mansioni

• Coordinate care for long-term care residents using the ENNOBLE CARE EMR.
• Maintain regular on-site engagement to support care plans and transitions.
• Collaborate with Ennoble Care providers within the long-term care setting.
• Serve as liaison between providers, facility staff, residents, and families.
• Communicate patient needs and changes to the clinical team promptly.
• Document patient interactions and care coordination activities.
• Follow established workflows and escalation protocols.
• Contribute to strong facility partnerships and team collaboration.
