Job summary

Your Health is seeking a Clinical Coordinator to support our patient care operations across clinics and facilities. The role includes chart preparation, scheduling of visits, and logging chronic care management activities, while providing provider and patient support in a fast-paced, value-based care environment.

This full-time, 8-hour salary-based position (8:00 AM–5:00 PM, Monday–Friday) involves working daily at our facilities and travel between offices as needed to maintain seamless care

Qualifications

• Must be a CMA or licensed nurse in good standing with the appropriate board.
• Minimum 1 year experience in a healthcare setting preferred.
• Must hold and maintain current CPR certificate.
• High school diploma or equivalent required.
• Strong interpersonal and communication skills under all conditions.
• Ability to foster a cooperative work environment and work as a team player.
• Basic computer knowledge.
• Must be able to travel between offices and facilities.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure documentation, test results, and diagnoses are accessible for the provider; update Navina as needed.
• Maintain accurate up-to-date patient records including demographics and insurance details.
• Ensure confidentiality and privacy compliance; verify consent forms are in patient charts.
• Reconcile medications as part of care coordination.
• Schedule patient appointments with providers and care team members.
• Communicate with patients via email, portal, phone, text, or in person; provide pre-appointment instructions.
• Document and log care management activities accurately.
