myPlace Health seeks a Behavioral Health Manager to lead our Social Work team within the PACE program, delivering holistic, person-centered care to high‑risk seniors in home and community settings. You will mentor social workers and counselors, align care with regulatory standards, and drive quality improvements.

Qualifications

• MSW from a CSWE-accredited program with active CA LCSW license.
• At least 5 years in progressive social work roles, with 3+ in leadership/management.
• Experience in healthcare, PACE, or senior-care settings preferred.
• Strong strategic thinking and decision-making.
• Excellent communication and people leadership skills.

Responsibilities

• Lead Social Work dept with clear goals and quality standards aligned with PACE.
• Supervise and mentor Social Workers and Mental Health Counselors.
• Manage team schedules, onboarding, and performance management.
• Use analytics and KPIs to guide service delivery improvements.
• Ensure timely biopsychosocial assessments and individualized care plans.
• Refine workflows and policies for consistent, participant-centered care.
• Support high-acuity cases, crisis interventions, and transitions of care.
• Ensure CMS, DHCS, and PACE regulatory compliance.
