Job summary

myPlace Health in Los Angeles, CA is hiring a Social Worker (MSW) for our PACE program. This full-time, onsite role focuses on psychosocial assessments, care planning, and care coordination with an interdisciplinary team to support elderly participants and their families.

The ideal candidate has an MSW, 2+ years of relevant experience, and CPR/First Aid. Bilingual abilities are a plus, and the role involves close collaboration with caregivers and community resources.

Qualifications

• MSW from an accredited university.
• CPR and First Aid certification (or obtainable within 30 days).
• Minimum 2 years of psychosocial assessments, care planning, and case management.
• Experience with frail or elderly populations preferred.
• California driver’s license with clean driving record.

Responsibilities

• Lead psychosocial assessments and develop individualized care plans.
• Coordinate transitions and care across IDT and settings.
• Assist with Medi-Cal applications and regulatory communications.
• Provide group counseling and support emotional well-being.
• Document interactions in the EMR promptly and accurately.
• Support discharge planning and community resource connections.
